*Printable Registration Form is in Adobe PDF format. You will need Adobe Reader . - Confirmation: Confirmation will be sent two weeks prior to weekend I.
- Refunds and cancellations: Cancellations received two weeks prior to weekend I will be refunded except for $100 deposit. You may transfer your registration to another person or a different date only by directly contacting the Excellence Training, LLC office prior to the start date. Excellence Training, LLC reserves the right to cancel or modify any social work continuing education course advertised.
